Birth certificates of applicant(s) and their mother(s).
Parents' Ketubas
Ketuba from parents' wedding (Jewish marriage certificate issued by a Rabbi).
Parents' Marriage Certificate
Legal marriage certificate issued at parents' wedding.
Two Testimonies
Confirmation of Single Status for the applicant(s) to be signed by two Jewish persons, neither related to each other nor to the applicant(s).
Second MARRIAGE
In case of second marriage, Gett certificate or Death certificate.
